Questionnaire Templates
Create, clone, and manage questionnaire definitions. Templates that have been used are locked to protect existing responses.
Download CSV Template
Download a CSV to edit offline. Uploading a CSV always creates a new template — it does not update the original.
The CSV file contains questions only. When you re-import, set Offering on the import form (defaults to General Discovery). If you exported an existing template, match its offering on import or edit the template after import.
Make your changes in Excel or another spreadsheet tool, then use Upload CSV to import as a new template. Used templates stay locked; re-import is the safe way to create a modified copy.
