Overview
Lead Quote Questionnaire is JK Seva’s general-purpose discovery tool for sales teams. Use it to capture customer requirements, share them securely for review, export structured results, and email completed questionnaires as file attachments — across any JK Seva offering, not just one product line.
Each engagement has two layers:
- Template — defines the sections and questions on the form.
- Offering — labels the type of engagement (for example General Discovery, Managed Services, or UKG Solutions) for customer communications and record context.
Reusable questionnaire definitions with sections and questions. Different templates can support different discovery flows.
Identifies the JK Seva offering for each template or questionnaire instance. Used in emails, dashboard context, and exports.
Individual filled-out questionnaires for a specific customer engagement.
Secure URLs that let customers review and update their responses.
Email a completed questionnaire as an HTML, text, Markdown, or JSON attachment to any recipient. See Share Questionnaire.
Getting started
Open the app and sign in with the credentials provided by an admin.
From the dashboard, click New Questionnaire and select a template. Only active (non-archived) templates appear in the picker. Pick the template whose questions fit this discovery — the offering label is set separately on the next step.
On the Start page, enter Customer Name and Questionnaire Name. Set Offering to match the engagement (defaults from the template). See Offering for the full list and how it is used.
Complete discovery questions during your call. Use section pills, Previous, or Next to move between topics.
On the Review step, scan unanswered items, then click Complete & Save when ready.
Send the questionnaire to the customer for review, download a CSV from the dashboard or completion screen, or use Share Questionnaire to email a completed questionnaire as a file attachment. See Share Questionnaire.
Dashboard
The dashboard lists every saved questionnaire instance. All signed-in users can see all questionnaires; use filters to narrow the list.
- Search — filter by customer name, representative, questionnaire name, offering, status, or date.
- Show only mine — list only questionnaires where you are the representative.
- Template column — shows which questionnaire template each instance was created from.
- Offering column — shows the engagement offering for each instance (for example General Discovery, Data Migration, or custom text when Other was selected).
- Sent / Opened — shows how many customer links were sent and how many times they were opened.
- Actions — Open, Send, CSV download, and Delete (for questionnaires you own, or any questionnaire if you are an admin).
Questionnaire templates
Templates define the structure of a questionnaire — name, sections, questions, field types, and order. Each saved questionnaire is an instance created from one template.
Different templates can support different discovery flows (for example a short general intake vs. a detailed technical questionnaire). The Offering field on a template identifies the engagement type for communications — it does not change which questions appear. To use different questions, choose or create a different template. See Offering.
Managing templates
Open Templates in the header to:
- Create a new blank template and add sections/questions.
- Clone any template (including locked or archived ones) to make an editable copy named “Copy of …”.
- Edit templates that have not been used yet.
- Archive / Unarchive templates to hide or restore them in the new-instance picker.
- Mark as Active when a draft template is ready for use.
- Delete unused templates that have never been used (no instances or customer links).
- Permanently Delete (admins, archived templates only) — remove the template and all related questionnaire instances and customer links. See Housekeeping.
Template status
- Offering — default engagement label for new questionnaires created from this template. Edit on the template page; see Offering.
- Draft — editable, not yet marked active.
- Active — available for new questionnaire instances.
- Locked — at least one instance was created or a customer link was sent; structure cannot be changed in place.
- Archived — hidden from the new-instance picker but still works for existing instances and customer links until permanently deleted.
The original Lead Quote Questionnaire is the first template, migrated automatically from the legacy question set.
Download CSV Template
On the Templates page, click Download CSV Template to open a download panel with two choices:
- Empty starter template — headers plus example rows to build a new questionnaire from scratch.
- Existing template — export a copy of a selected template’s sections and questions for offline editing.
Choose an option, pick a template if exporting an existing one, then click Download CSV.
Import from CSV
On the Templates page, click Upload CSV to import a spreadsheet and create a new editable draft template. Each row becomes one question. Import always creates a new template — it never updates an existing template in place.
- Click Download CSV Template and choose the empty starter or an existing template export.
- Fill in or modify sections and questions using the column guide below.
- Upload the file and optionally set a template name before clicking Import Template.
How CSV columns map to the questionnaire
The columns in your spreadsheet control what users see on the form:

Invalid files are rejected with clear errors (duplicate keys, missing columns, invalid types, invalid priority).
Housekeeping test templates and data
Use these workflows to clean up test templates and related data without affecting production questionnaires you still need.
- Archive first — hides a template from the new-instance picker while keeping existing instances and links working.
- Delete (unused templates only) — removes a template that has never been used. Available when the template has no instances.
- Permanently Delete (admins, archived templates only) — removes the archived template and all related questionnaire instances and customer links. Intended for test cleanup; this is irreversible.
When you click Permanently Delete, the app shows an impact preview listing questionnaires and customer links that will be removed before you confirm.
- Archive — template hidden from new questionnaires; existing data stays.
- Permanent delete — template, questionnaires, and customer links are removed together. Cannot be undone.
Offering
The Offering field labels the type of JK Seva engagement for customer communications and record context. It helps personalize send-to-customer emails without hard-coding one product line into the app.
What Offering does and does not do
- Does — appear in the dashboard Offering column, CSV exports, and customer email subject/body text (via the
{{offering}}placeholder). - Does not — change which questions appear on the form. Questions always come from the selected template.
Where to set Offering
- Template edit — set a default offering for all new questionnaires created from that template.
- Questionnaire Start page — new instances inherit the template default; you can change it before continuing if the engagement differs.
Selectable offerings
Choose from the list below. The neutral default is General Discovery (used when nothing else is set).

Customer communications
On Send to Customer, default subject lines and the email body can include {{offering}}, which is replaced with the questionnaire’s offering label when the email is sent. See Send to Customer.
Filling a questionnaire
Required fields (Start page only)
- Representative — set automatically from your signed-in username.
- Customer Name — legal or commonly used customer name.
- Questionnaire Name — short label for this engagement.
Offering (Start page)
Select the JK Seva offering for this engagement. The value defaults from the template but can be changed on the Start page before you continue. If you choose Other, enter custom offering text — it is required and appears in customer communications instead of a preset label.
Discovery questions
All discovery questions are optional. “Unknown” or blank is acceptable when details are not yet available.
Navigation
- Section pills — click any section in the progress bar to jump directly.
- ‹ Previous / Next › — move one section at a time.
- Save Progress — manually save at any time. Success message: “Your updates have been saved.”
- Complete & Save — on the Review step, marks the questionnaire complete.

Section pills show how complete each section is:

Unanswered questions
Questions without a response show an amber Not answered badge so you can spot gaps quickly on the Review step.
Auto-save
After the Start page is complete, responses auto-save in the background. The save indicator shows saving, saved (with PST timestamp), or an error.
Send to Customer
Share a secure link so the customer can review and update their questionnaire responses.
- Click Send on the dashboard or Send to Customer on the questionnaire toolbar.
- Enter the customer’s first name, last name, and email address.
- Choose a subject line and edit the email body if needed. Default templates use the questionnaire’s Offering so messages read naturally — for example “your General Discovery questionnaire” or “your Managed Services requirements”.
- The system creates a secure link (
/customer/q/{token}) and sends the email when SMTP is configured. - When the customer email is sent successfully, the representative also receives a confirmation email with questionnaire details, next steps, the customer link, and a link to review responses in JK Seva. The subject begins with
[JKS Questionnaire]for easy inbox filtering. - Customers see a simplified page with the same sections and questions. They can save progress and click Finish Review on the last section to submit.
- When a customer finishes, their responses are saved, the link is marked completed, and the representative receives an email with a link to the results (requires SMTP).
- Sending a link also locks the template so its structure cannot change afterward.
Email placeholders
The send form replaces these tokens automatically when the email is sent:

Use {{offering}} in the subject or body to personalize communication for any JK Seva offering without editing the app’s default wording for each product line.
Managing customer links
From the send page or the admin Customer Links page you can complete, revoke, re-activate, or delete links. Use Show only mine to filter links you sent.
Exporting results
Download a CSV from the dashboard actions column or from the completion screen after submit.
The export includes:
- Representative, customer name, questionnaire name, and offering
- Question labels, keys, sections, and types
- Responses and answered / not answered status
CSV exports use the template that was active when the instance was created, so cloned or custom templates export correctly.
Administration
Admins access the Admin page from the header to manage users, shortcuts, and settings:
- Users — create accounts with full name and email (used as login), assign user or admin role, reset passwords. (Admin)
- Templates — shortcut to the template management area. (Templates)
- Audit Log — search recent application activity. (Audit Log)
- Customer Links — view all access links, complete, revoke, re-activate, or delete. (Customer Links)
- SMTP Settings — configure outbound email and run a connection test. (SMTP Settings)
- API key — set the key used by external integrations (see the API page).
Admins can delete any saved questionnaire from the dashboard. Regular users can delete questionnaires they created.
Audit Log
The Audit Log page records important operational actions for short-term support and review. It is not permanent compliance storage.
Recorded actions include sign-in (success and failure), user create/update/delete, API key and SMTP changes, SMTP tests, questionnaire create/delete, template create/clone/archive/unarchive/purge, CSV template imports (template.import_csv), customer link send/fail/complete/revoke/reactivate, questionnaire share (questionnaire.share), and audit-log purges.
- Search and filter — free-text search plus filters for action, actor, target type, and result. Quick buttons find CSV imports and template purges.
- CSV import tracking — imports are logged separately from normal template creates, with template name and question/section counts.
- Export before purge — download a CSV of entries that will be removed (all entries, or entries older than a chosen number of days).
- Purge audit log — remove old or all audit entries when housekeeping is needed.

Frequently asked questions
What is Share Questionnaire?
Share Questionnaire emails a completed questionnaire as a file attachment (HTML, plain text, Markdown, or JSON) to any recipient. Any signed-in representative or admin can use it from the header navigation. See Share Questionnaire.
Why don’t I see a questionnaire in the Share list?
Only questionnaires with status Complete appear. Open the questionnaire, finish the Review step, and click Complete & Save first. Also check Show only mine and the search box — your filters may be hiding the row.
What attachment formats are available when sharing?
HTML (.html), plain text (.txt), Markdown (.md), and JSON (.json). Choose HTML for readable reports, plain text for maximum compatibility, Markdown for tools that render MD, and JSON for structured technical handoff.
How is Share Questionnaire different from Send to Customer?
Send to Customer emails a secure link so the customer can review and update responses in the app. Share Questionnaire emails a static file attachment of a completed questionnaire to any address — no customer link, no in-app editing for the recipient.
Can this be used for offerings other than UKG?
Yes. Lead Quote Questionnaire is a general discovery tool for multiple JK Seva offerings. Select the appropriate offering (Infor Solutions, Data Migration, Managed Services, and others) or use General Discovery when the engagement is not product-specific yet. UKG Solutions is one option among several — not the product identity.
What does the Offering field do?
Offering labels the type of JK Seva engagement for customer communications and record context. It appears on the dashboard, in CSV exports, and in send-to-customer emails (via {{offering}}). It does not change which questions appear — those come from the template.
What is General Discovery?
General Discovery is the neutral default offering when no specific product line applies. Use it for exploratory conversations or mixed engagements. Email templates still read naturally — for example “your General Discovery questionnaire”.
Does changing the Offering change the questionnaire questions?
No. Offering only affects labels and customer communication. To use different questions, select a different template or create/clone/import a template with the structure you need.
Where does Offering appear in customer emails?
Default subject lines and the email body on Send to Customer include offering context — for example “your {{offering}} questionnaire”. The {{offering}} placeholder is replaced with the questionnaire’s offering label (such as General Discovery or Managed Services) when the email is sent. You can edit the subject and body before sending.
Why can't I edit a template?
Templates become locked once a questionnaire instance is created from them or a customer link is sent. Clone the template to make an editable copy.
Why doesn't my archived template appear when creating a new questionnaire?
Archived templates are intentionally hidden from the new-instance picker. Unarchive the template on the Templates page, or clone it to create a new active version.
What's the difference between Save Progress and Complete & Save?
Save Progress saves your work as a draft so you can continue later. Complete & Save on the Review step marks the questionnaire as complete.
Can I create multiple questionnaires for the same customer?
Yes. Start a new questionnaire with a different questionnaire name for each engagement or discovery phase.
The customer link says it is unavailable. What happened?
The link may have been revoked, completed, or deleted. Check the Customer Links admin page or send a new link from the questionnaire send page.
Who can see my questionnaires?
All signed-in JK Seva users can view every saved questionnaire on the dashboard. Use Show only mine to filter to your own entries.
Why can't I use a custom priority value in CSV import?
The import validator only accepts critical, high, medium, and low — exact lowercase spelling. This keeps sorting and reporting consistent. Leave priority blank to default to medium.
What is the difference between archive and permanent delete?
Archive hides a template from the new-questionnaire picker but keeps existing questionnaires and customer links working. Permanently Delete (admins, archived templates only) removes the template and all related questionnaire instances and customer links. Permanent delete is for test/housekeeping cleanup and cannot be undone.
What happens when I permanently delete an archived template?
The app shows an impact preview first. If you confirm, the archived template is removed along with every questionnaire instance created from it and every related customer link — including saved answers and send history. Production templates you still need should stay archived, not permanently deleted.
Can I download an existing template as CSV and modify it?
Yes. On Templates, click Download CSV Template, choose Existing template, select the template, and download. Edit offline, then Upload CSV to create a new template. The original is not updated in place.
Where can I see whether a template was imported from CSV?
Admins: open Audit Log and filter by action template.import_csv, use the CSV imports quick button, or search by template name or id. CSV imports are logged separately from normal template.create actions.
Does purging the audit log delete questionnaires or templates?
No. Audit log purge removes operational history entries only. Questionnaires, templates, customer links, users, and SMTP settings are unaffected. Export to CSV first if you want a copy of the entries being cleared.
Tips for sales calls
- Pick the template whose questions fit the call, then set Offering to match the engagement type for follow-up emails.
- Start the questionnaire during the first discovery call and fill in what you learn live.
- Use section pills to jump between topics without losing progress.
- On the Review step, scan Not answered badges to build your follow-up question list.
- Send the customer link after the call so they can fill in details you could not capture live.
- After a questionnaire is complete, use Share Questionnaire to email the results as an attachment to a colleague, partner, or technical contact who needs the full record.
- Create a new questionnaire name for a separate engagement with the same customer.
