Sign in Use your JK Seva email address and password. Change your password anytime from the Password link in the header. Your signed-in username is recorded as the representative on new questionnaires.

Getting started

Sign in

Open the app and sign in with the credentials provided by an admin.

Choose a template

From the dashboard, click New Questionnaire and select a template. Only active (non-archived) templates appear in the picker.

Enter customer details

On the Start page, enter Customer Name and Questionnaire Name. Both are required before you can continue or save.

Work through sections

Complete discovery questions during your call. Use section pills, Previous, or Next to move between topics.

Review and complete

On the Review step, scan unanswered items, then click Complete & Save when ready.

Send or export

Send the questionnaire to the customer for review, or download a CSV from the dashboard or completion screen.

Dashboard

The dashboard lists every saved questionnaire instance. All signed-in users can see all questionnaires; use filters to narrow the list.

  • Search — filter by customer name, representative, questionnaire name, status, or date.
  • Show only mine — list only questionnaires where you are the representative.
  • Template column — shows which questionnaire template each instance was created from.
  • Sent / Opened — shows how many customer links were sent and how many times they were opened.
  • Actions — Open, Send, CSV download, and Delete (for questionnaires you own, or any questionnaire if you are an admin).
Tip Use a descriptive questionnaire name such as “Initial Discovery Q3 2026” so you can distinguish multiple engagements with the same customer.

Questionnaire templates

Templates define the structure of a questionnaire — name, sections, questions, field types, and order. Each saved questionnaire is an instance created from one template.

Managing templates

Open Templates in the header to:

  • Create a new blank template and add sections/questions.
  • Clone any template (including locked or archived ones) to make an editable copy named “Copy of …”.
  • Edit templates that have not been used yet.
  • Archive / Unarchive templates to hide or restore them in the new-instance picker.
  • Mark as Active when a draft template is ready for use.

Template status

  • Draft — editable, not yet marked active.
  • Active — available for new questionnaire instances.
  • Locked — at least one instance was created or a customer link was sent; structure cannot be changed.
  • Archived — hidden from the new-instance picker but still works for existing instances and customer links.
Locked templates If a template has already been used, you cannot edit it. Clone it instead to make changes without affecting existing responses.

The original Lead Quote Questionnaire is the first template, migrated automatically from the legacy question set.

Import from CSV

  • Click Download CSV Template to get a correctly formatted starter file with example questions.
  • Fill in sections and questions using columns: section, sectionId, key, label, type, priority, required, options, helpText, order.
  • Click Upload CSV to import a file and create a new editable draft template.
  • Invalid files are rejected with clear errors (duplicate keys, missing columns, invalid types).

Filling a questionnaire

Required fields (Start page only)

  • Representative — set automatically from your signed-in username.
  • Customer Name — legal or commonly used customer name.
  • Questionnaire Name — short label for this engagement.

Discovery questions

All discovery questions are optional. “Unknown” or blank is acceptable when details are not yet available.

Navigation

  • Section pills — click any section in the progress bar to jump directly.
  • ‹ Previous / Next › — move one section at a time.
  • Save Progress — manually save at any time. Success message: “Your updates have been saved.”
  • Complete & Save — on the Review step, marks the questionnaire complete.

Section progress colors

Section pills show how complete each section is:

Complete all questions answered   Partial some answered   Empty none answered

Unanswered questions

Questions without a response show an amber Not answered badge so you can spot gaps quickly on the Review step.

Auto-save

After the Start page is complete, responses auto-save in the background. The save indicator shows saving, saved (with PST timestamp), or an error.

Send to Customer

Share a secure link so the customer can review and update their questionnaire responses.

  • Click Send on the dashboard or Send to Customer on the questionnaire toolbar.
  • Enter the customer’s first name, last name, and email address.
  • Choose a subject line and edit the email body if needed.
  • The system creates a secure link (/customer/q/{token}) and sends the email when SMTP is configured.
  • Customers see a simplified page with the same sections and questions. They can save progress and click Finish Review on the last section to submit.
  • When a customer finishes, their responses are saved, the link is marked completed, and the representative receives an email with a link to the results (requires SMTP).
  • Sending a link also locks the template so its structure cannot change afterward.

Managing customer links

From the send page or the admin Customer Links page you can complete, revoke, re-activate, or delete links. Use Show only mine to filter links you sent.

SMTP not configured? The link is still created. Copy it from the send confirmation and email it to the customer manually.

Exporting results

Download a CSV from the dashboard actions column or from the completion screen after submit.

The export includes:

  • Representative, customer name, and questionnaire name
  • Question labels, keys, sections, and types
  • Responses and answered / not answered status

CSV exports use the template that was active when the instance was created, so cloned or custom templates export correctly.

Administration

Admins access the Admin page from the header to manage:

  • Users — create accounts with full name and email (used as login), assign user or admin role, reset passwords.
  • Templates — shortcut to the template management area.
  • Customer Links — view all access links, complete, revoke, re-activate, or delete.
  • SMTP Settings — configure outbound email and run a connection test.
  • API key — set the key used by external integrations (see the API page).

Admins can delete any saved questionnaire from the dashboard. Regular users can delete questionnaires they created.

Frequently asked questions

Why can't I edit a template?

Templates become locked once a questionnaire instance is created from them or a customer link is sent. Clone the template to make an editable copy.

Why doesn't my archived template appear when creating a new questionnaire?

Archived templates are intentionally hidden from the new-instance picker. Unarchive the template on the Templates page, or clone it to create a new active version.

What's the difference between Save Progress and Complete & Save?

Save Progress saves your work as a draft so you can continue later. Complete & Save on the Review step marks the questionnaire as complete.

Can I create multiple questionnaires for the same customer?

Yes. Start a new questionnaire with a different questionnaire name for each engagement or discovery phase.

The customer link says it is unavailable. What happened?

The link may have been revoked, completed, or deleted. Check the Customer Links admin page or send a new link from the questionnaire send page.

Who can see my questionnaires?

All signed-in JK Seva users can view every saved questionnaire on the dashboard. Use Show only mine to filter to your own entries.

Tips for sales calls

  • Start the questionnaire during the first discovery call and fill in what you learn live.
  • Use section pills to jump between topics without losing progress.
  • On the Review step, scan Not answered badges to build your follow-up question list.
  • Send the customer link after the call so they can fill in details you could not capture live.
  • Create a new questionnaire name for a separate engagement with the same customer.
